Survey Forms

The IBMPFD Symptoms survey form identifies a number of symptoms of IBMPFD.  Completing the form allows researchers to establish a data base to understand rates of degeneration and other factors.  The completed form may either be directly e-mailed to Dr. Weihl and Dr. Kimonis; or may be printed an mailed.

The IBMPFD Quality of Life survey form identifies a number of ways the symptoms of IBMPFD can affect one's life.  Completing the form allows researchers to establish a data base to understand the correlation between life style and the effects of IBMPFD.  The completed form may either be directly e-mailed to Dr. Kimonis; or may be printed and mailed.
